Procedure for completing professional internships – medical program

Hospitals with which the University has an agreement:

1. The student pays the fee for professional internships in accordance with the fee required by the hospital (to the student’s individual account).

2. The student provides the Internship Plenipotentiary with confirmation of payment for internships at the given hospital (electronic version via email is acceptable) and the required documents (listed below).

Required documents to be submitted before starting the internship:

  • OC and NNW insurance, including post-exposure prophylaxis, valid for the entire calendar year
  • result of testing for Salmonella carriage (medical certificate for sanitary-epidemiological purposes) (once during the entire course of studies)
  • hepatitis B vaccination (once during the entire course of studies)
  • copy of a certificate from an occupational medicine physician

After presenting the documents, the Plenipotentiary stamps the internship referral completed by the student (2 copies of referral).

3. With the referral, the student reports to the hospital to arrange the date of the internship and submit documentation required by the hospital.

After completing the internship, the following must be submitted:

  • completed internship logbook with confirmation of completion
  • internship referral

Hospitals based on individual agreements:

1. The student applies to the Hospital Director with a request for consent to complete the internship (it is recommended to provide the agreement template prepared by AWSB – if the hospital provides a different contract than the university agreement, it must be sent in advance for approval).

2. With written consent from the Hospital Management, printed documents (2 completed originals – not copies – of the internship referral and 2 completed agreements), and required documents (listed below), the student reports to the Internship Plenipotentiary to obtain signatures.

Required documents that must be submitted to the Plenipotentiary for professional internships before starting the internship:

  • OC and NNW insurance (including post-exposure prophylaxis)
  • Salmonella test result
  • hepatitis B vaccination
  • occupational medicine certificate copy

3. The student delivers the obtained documents to the hospital where consent was granted.

After completing the internship, the following must be submitted:

  • agreement
  • completed internship logbook
  • internship referral
